How do I create a new PREFcard from scratch?

Log-in as "Card Editor", Click the hamburger lines, click "Surgeons", click the Surgeon you wish to create a new PREFcard from scratch.

Click "+Create", type the name of the PREFcard in the "Procedure Name" Box then click "Create PREFcard".  

A new form will pop-up in edit mode. The new form is now ready for editing.
