How do I create a new PREFcard using another card as the starting template?

Log-in as "Card Editor", Click the hamburger lines, click "Surgeons". 

Click the Surgeon and specific procedure you wish to use as a starting template. 

Click "Actions" click Copy.  

Select the name of the Surgeon and the Facility you wish to create a PREFcard for and then select the "Procedure" then click "Copy PREFcard".
